Job Description:
Touchstone Communities is seeking a dedicated and
detail-oriented RN MDS Coordinator to join our healthcare team.
This position will be integral in enhancing the quality of care for
our senior residents by accurately assessing their needs and
ensuring their care plans are comprehensive and tailored. Working
within our supportive environment, the ideal candidate will
coordinate and oversee the entire MDS assessment process, ensuring
compliance with federal and state regulations while aligning with
our mission to provide personalized, compassionate care. Conduct
and coordinate the Minimum Data Set (MDS), Care Area Assessments
(CAAs) and Care Plans in accordance with CMS regulations
Collaborate with a multidisciplinary team to provide timely and
accurate assessments Ensure the transmission of MDS to the state
database meets all required deadlines Utilize MDS data to inform
and develop individualized care plans that meet the medical,
emotional, and psychosocial needs of the residents Train and
support nursing staff regarding changes in the MDS process Monitor
and audit clinical records on a regular basis to ensure
documentation is maintained and complies with applicable laws and
regulations Participate in quality assurance practices to ensure
quality care delivery in accordance with Touchstone Communities'
standards Strong knowledge of MDS 3.0 Familiarity with nursing
practices and procedures Excellent organizational and time
management skills Ability to lead and educate nursing staff Strong
analytical and problem-solving skills Proficient in healthcare
management systems and software 401k Valid Registered Nurse (RN)
license Certified Resident Assessment Coordinator (CRAC) preferred
Ongoing training and development related to MDS processes and
updates
