Reports To: Chief Medical Officer Location: Caroline Clinic,
Cathedral Clinic, Reed Family Clinic, and Street Medicine Outreach
Overview The Advanced Practice Provider (Physician Assistant or
Nurse Practitioner) provides high-quality, trauma-informed, and
patient-centered care to individuals and families experiencing
homelessness, housing instability, and complex medical, behavioral,
and social needs. Practicing within a Federally Qualified Health
Center (FQHC) and under the supervision of a licensed physician,
this position serves in both traditional clinic settings and
community-based street medicine environments. The Provider plays a
key role in improving access to care for underserved populations by
delivering comprehensive medical services, advancing health equity,
and supporting integrated models of care that address physical,
behavioral, and social needs. Duties and Responsibilities Clinical
Care (Clinic and Street-Based Settings) Conduct comprehensive
patient assessments, including history-taking, physical exams,
screenings, and diagnostic evaluations. Diagnose and manage acute
and chronic conditions such as diabetes, hypertension, asthma, HIV,
hepatitis C, and behavioral health concerns. Prescribe medications,
therapies, and other interventions in accordance with licensure and
scope of practice. Order, interpret, and act on diagnostic tests,
labs, and imaging studies to inform patient care. Provide
preventive care and health maintenance, including immunizations,
well visits, reproductive health, and health education. Deliver
care using trauma-informed, harm-reduction, and culturally
responsive approaches that build trust with individuals in
nontraditional settings. Participate in outreach and mobile health
operations, providing care in encampments, shelters, day centers,
and other field environments. Implement safety planning and crisis
intervention in collaboration with behavioral health and outreach
teams. Care Coordination and Team Collaboration Collaborate with a
multidisciplinary care team—including physicians, nurses,
behavioral health providers, care coordinators, and community
health workers—to ensure whole-person, integrated care. Coordinate
referrals to specialty care, diagnostics, behavioral health, and
substance use services; ensure follow-up and continuity of care.
Participate in clinical and street medicine huddles,
interdisciplinary case conferences, and quality improvement
initiatives. Partner with community agencies, shelters, hospitals,
and housing providers to streamline services and reduce barriers to
care. Participate in community outreach, patient engagement, and
health education events as needed. Compliance, Documentation, and
Quality Improvement Maintain timely, accurate, and complete
documentation in the Electronic Health Record (EHR), in accordance
with organizational policies and regulatory standards. Participate
in ongoing quality improvement, performance measurement, and UDS
reporting as part of FQHC operations. Adhere to all applicable
laws, regulations, and practice standards (e.g., HIPAA, OSHA, HRSA,
FTCA). Engage in continuing education to stay current on clinical
guidelines and emerging practices relevant to community-based care.
Comply with all FQHC reporting requirements, including UDS data
collection and quality metrics. Support HHH Mission Demonstrates a

health disparities and advance systems-level change. Minimum
Qualifications Graduate of an accredited Physician Assistant or
Nurse Practitioner program Current license to practice in the State
of Texas Current national certification (NCCPA, ANCC, or AANP) DEA
license and Basic Life Support (BLS) certification At least 1–2
years of clinical experience in a primary care, urgent care, or
community-based setting Experience with Electronic Health Records
(EHR), preferably EPIC Strong clinical judgment, adaptability, and
communication skills Preferred Qualifications Experience working
with medically underserved populations, especially individuals
experiencing homelessness, housing instability, and/or co-occurring
behavioral health conditions Experience in a Federally Qualified
Health Center (FQHC) or similar safety-net setting Familiarity with
trauma-informed care, harm reduction, and integrated care models
Bilingual in English and Spanish Experience with MAT, HIV, or
chronic disease management programs HHH values lived experience as
