Job Description JOB SUMMARY Performs responsible clinical care
in collaboration with the unit physician and under approved
protocols, manages the medical aspects of care in the observation,
assessment, diagnosis, intervention, evaluation, rehabilitation,
care and counsel, and health teachings of persons who are ill,
injured or infirm or experiencing changes in normal health
processes; and in the promotion and maintenance of health of
prevention of illness in a clinical setting. PHYSICIAN ASSISTANT
EDUCATION & EXPERIENCE Minimum Qualifications: - Completion of a
physician assistant program accredited by the Accreditation Review
Commission on Education for the Physician Assistant (ARC-PA) - No
experience required LICENSES, REGISTRATIONS OR CERTIFICATIONS
Required: - Certification by the National Commission on
Certification of Physician Assistants (NCCPA) - Licensed Physician
Assistant by the Texas Physician Assistant Board/Texas Medical
Board (TPAB/TMB) Current students who have not yet completed their
physician assistant program or recent graduates who have not yet
obtained their NCCPA certification or TPAB/TMB physician assistant
license are eligible to apply for this position. However, official
employment is contingent upon the successful completion of an
accredited physician assistant program, NCCPA certification and
TPAB/TMB physician assistant licensure. NURSE PRACTITIONER
EDUCATION & EXPERIENCE Minimum Qualifications: - Completion of a
graduate level (or higher) nurse practitioner program accredited by
the Accreditation Commission for Education in Nursing (ACEN) or the
Commission on Collegiate Nursing Education (CCNE) - No experience
required LICENSES, REGISTRATIONS OR CERTIFICATIONS Required: -
Certification by the American Nurses Credentialing Center (ANCC) or
American Academy of Nurse Practitioners (AANP) - Licensed
Registered Nurse (RN) by the Texas Board of Nursing (TBON) -
Licensed Advanced Practice Registered Nurse (APRN) in a nurse
practitioner clinical area of specialization by the Texas Board of
Nursing (TBON) Current RNs who have not yet completed their nurse
practitioner program or recent graduates who have not yet obtained
their ANCC/AANP certification or APRN nurse practitioner license
are eligible to apply for this position. However, official
employment is contingent upon the successful completion of an
accredited nurse practitioner program, ANCC/AANP certification and
APRN nurse practitioner licensure. Job Duties: - Independently
manage outpatient nephrology and dialysis patients within the scope
of practice and in collaboration with nephrologists. - Work
independently to full scope of practice in compliance with the
Texas Nurse Practice Act with acknowledgement of off-site
collaborating physician availability for consultation and
collaboration. - Conduct comprehensive patient evaluations,
including history-taking, physical examinations, and
ordering/interpreting diagnostic tests. - Develop and implement
evidence-based treatment plans for acute and chronic kidney
disease, hypertension, electrolyte disorders, and dialysis-related
complications. - Prescribe medications and therapies following best
practice guidelines and regulatory standards. - Provide education
and counseling to patients and families regarding disease
processes, treatment plans, and lifestyle modifications. -
Collaborate with multidisciplinary teams, including physicians,
nurses, dietitians, and social workers, to ensure optimal patient
care. - Participate in quality improvement projects aimed at
enhancing patient outcomes and operational efficiency. - Engage in
teaching and mentoring opportunities for APPs, nurses, and other
healthcare professionals. - Maintain accurate documentation in
electronic medical records (EMR) to ensure compliance with
healthcare regulations and quality standards. - Be an active
participant in the Nephrology clinic team; work with faculty to
identify opportunities to shift more stable patients to an APP to
improve access for new patients, assist with triaging patients, and
other clinic needs as time allows. Schedule & Clinic Locations: -
This position will be centralized around our Webster patient
population. This position will be deployed to a clinic in Webster
region and would round on dialysis patients in facilities on the
mainland. - This APP will round on dialysis patients in a dialysis
facility every other week. - This Advanced Practice Provider will
be responsible for nine half-day clinic sessions per week on
non-dialysis weeks and four to six half-day clinic sessions per
week on dialysis rounding weeks. - A typical clinic schedule will
be focused on follow-up patients who have been previously seen by a
faculty member. Once fully ramped up, the expectation is to see 8
patients per half-day session. - Skills & Experience: - Education:
Master’s or Doctorate in Nursing from an accredited Nurse
Practitioner program or Master of Physician Assistant Studies
(MPAS) - Certification: Board-certified as a Nurse Practitioner
(ANCC or AANP) or Physician’s Assistant (PA) - Licensure: Active or
eligible APRN or PA license in the state of Texas. - Ideal
candidates will have: - Previous experience practicing
independently in an outpatient setting. - Prior nephrology,
dialysis, or internal medicine experience. - Strong clinical
reasoning, exceptional communication, and teamwork skills with a
